Pexiganan advanced to Phase 3 evaluation as a topical agent for the treatment of infections associated with diabetic foot ulcers. In 1999, the US FDA denied approval of pexiganan after trial results revealed it was no more effective than treatments already available [2]. In 2016, it was reported that pexiganan had failed to meet the primary clinical endpoints in two further Phase 3 studies (NCT01594762 and NCT01590758) [4].

In bacteria, pexiganan disrupts the cell membrane by formation of toroidal pores, leading to cell death [2].
